With some help from Consultants, I’m designing an entry form box with entry note pads. Here is a photo of the sample box as it looks at the moment:
The text above the entry slot says:
“You may only enter the drawing once every calendar month. You will be contacted at the beginning of the next calendar month if you win. GOOD LUCK!”
